In the clinical area the MA/PBC, under the direction and supervision of a registered nurse and in accordance with the policies of Boston Health Care for the Homeless Program’s procedures, protocols, and standards of practice, participates in the delivery of patient care. In the administrative area the MA/PBC gathers patient information for registration, schedules appointments, prepares medical records, and files. The MA/PBC will also be responsible for patient appointment confirmation as well as enrolling BHCHP patients into health care plans.

Share fully in the delivery of clinical care directly on the streets of Boston, at Massachusetts General Hospital and Barbara McInnis House, as well as through home visits The NP or PA will be a member of a multidisciplinary team consisting of two internists, two psychiatrists, two NP/PAs, and a Case Manager.  All members of the team share fully in offering continuity of medical, psychiatric, and addictions care to a panel of “rough sleepers,” individuals who have avoided shelters and lived chronically on the streets of Boston.  The service model is predicated upon the delivery of integrated care to these vulnerable individuals on the Pine Street Inn Overnight van, through daytime street rounds, thrice weekly clinics at MGH, in our Barbara McInnis House, and even extending to house calls to those persons who have lived chronically on Boston’s streets and have been placed in housing.  The Street Team also visits patients admitted to MGH and BMC, as well as those in nursing and rest homes.

The nurse works as part of a collaborative team under a Patient Centered Medical Home (PCMH) Model, to provide nursing care to an underserved population. This collaboration includes daily communications and complex decision-making around both social and medical needs for our homeless patients. Communication occurs frequently with members of the care team, which includes Case Management, Medical Assistant, Front Desk Staff, Phlebotomy and Provider disciplines.   This position provides all nursing related activities in association with primary care and clinical operations including direct care and nursing case management activities, immunization and screening.  This position also allows for administrative and clinical responsibilities for the evening operation of the BMC clinic.
